Medical Overview

What is Varicocele?

A varicocele is an enlargement of the veins within the scrotum that drain blood from the testicle, similar to varicose veins in the legs. It occurs in around 15% of adult men, predominantly on the left side due to venous anatomy.

Prevalence in men: Around 15% of adult males

Grading of Varicocele

Grade 1 Varicocele

Small size, palpable only during the Valsalva maneuver (straining).

Grade 2 Varicocele

Medium size, palpable during physical examination without straining, not visibly noticeable.

Grade 3 Varicocele

Large size, easily visible and palpable through scrotal skin ("bag of worms" appearance).

Bilateral Varicocele

Present on both left and right sides of the scrotum.

Signs to Look For

Symptoms

  • Dull or aching discomfort in the scrotum
  • Feeling of scrotal heaviness or dragging sensation
  • Enlarged, visibly twisted scrotal veins
  • Difference in size between testicles (testicular atrophy)
  • Reduced sperm count, motility, or morphology
Contributing Factors

Causes

Physical & Medical Factors

Incompetent or malfunctioning venous valves in the spermatic cord • Elevated hydrostatic pressure in left testicular vein • Increased scrotal temperature

Clinical Evaluation

Diagnosis & Clinical Assessment

Physical examination standing and supine with Valsalva maneuver, paired with high-resolution Scrotal Color Doppler Ultrasound to evaluate reflux and vein diameter.

Frequently Asked Questions

Common Questions & Medical Clarity

QCan varicocele cause infertility?

It can affect sperm production and quality in some men, though not every man with varicocele will experience fertility problems.

QCan varicocele affect testosterone?

In some men, varicocele may be associated with reduced testosterone production.

QDoes every varicocele need surgery?

No. Small, painless varicoceles without fertility impairment can often be monitored and supported with conservative and Ayurvedic therapies.

QCan varicocele disappear on its own?

A varicocele generally does not disappear on its own. Treatment is determined by symptoms and semen parameters.
